    You do not need a referral from a physician to be seen in our office unless it is required by your insurance company. Please consult with your employer or insurance carrier if you are uncertain of the requirements.

    We currently participate with the following insurances: Blue Cross Blue Shield, Blue Care Network, Medicare, Medicare Advantage, Priority Health, Sault Tribe of Chippewa Indians Contract Health (with voucher), Little Traverse Band of Odawa Indians Contract Health (with a reference number), ASR-Physician's Care, Standard Tricare, Tricare for Life and Tricare Prime, Cofinity-PPOM, and Aetna.
  • WE DO NOT HAVE ANY CONTRACTUAL AGREEMENT WITH UNITED HEALTHCARE, CIGNA, MEDICAL MUTUAL, MERITIAN HEALTH, SEAFARERS OR ANY OF THEIR ASSOCIATED NETWORKS OR REPRICERS INCLUDING BUT NOT LIMITED TO: COALITION AMERICA, NPPN, IHP, PHS, HPO, VIANT, IHG, BEECH STREET, GLOBAL CLAIM RESOURCES OR ANY HEALTHSMART HOLDINGS.  ANY INFORMATION ON THEIR COMPANY WEBSITES OR WITHIN THEIR PROVIDER DATABASE CLAIMING A NETWORK RELATIONSHIP IS INCORRECT. 
  • Payment Policies:
    Our office accepts payment by personal check, cash, Mastercard or Visa. Payment for non-covered services, copays and deductible amounts are expected at the time of service.  See Pre-registration for full financial policy.
  • Allergy Injection Policies:
    Allergy injections are given on a walk-in basis on designated days and hours. The current injection schedule can be obtained by calling the Petoskey office at 231-487-6575 #5 or can be found on this site by clicking the announcements button. Allergy injection patients visiting our beautiful resort area are welcome with appropriate records from their personal physician/allergist. We require all immunotherapy patients to remain under observation by our staff for the twenty minute period following administration of the injection.
  • Minor Patients:
    Minor patients must be accompanied by a parent or guardian when medical services are rendered.
